Form Instructions

If you wish to have a block party, please fill out the following request form. We request that you make your block party request at least two weeks prior to the event. We will send you a confirmation letter via e-mail after we have reviewed and approved your request. The approval process typically takes 2-3 days. For more information, please see the Block Party Guidelines and Procedures noted above.

Block parties are required to adhere to the Centers for Disease Control and Prevention (CDC) and Illinois Department of Public Health (IDPH) requirements at the time of the event. Please note, requests for Police and Fire visits are subject to availability and COVID-19 best practices.
